Incoming 9th Graders

ALL MEDICAL FORMS WILL BE ONLINE DURING   2022-2023 SCHOOL YEAR

No paper forms will be accepted

1. Visit  www.homecampus.com and choose California state.
2. The website takes you step by step through your Athletes Clearance.
3. Your School will review the information you have submitted and Clear or Deny
   your student for participation.
   
4. An email notification will be sent when the student has been cleared for participation.

6. Make sure only an MD, DO, or PNP can fill out the physical form (no Chiropractor).   
   1. Be sure the doctor marks the clearance box
   2. Be sure the doctor writes the exam date and it needs to be within a year
   3. Be sure the physical form has a doctor's office Stamp.   
   4. **Clearance can take up to 5 working days**
